Patient & Family Rights
- The right to considerate the care with full respect of patient’s dignity regardless of the nationality, colour, age, sex, religion and disability.
- Right to select / change the doctor.
- Right to know them by name the physicians, nurses and staff members involved in the treatment.
- Right to request for a change or transfer to other hospital as per hospital policy.
- Right to avail treatment.
- Right to get safe environment during treatment.
- Right to be free from any form of physical, verbal abuse and harassment.
- Right to know the nature and inherent risk of any procedure to which the patient has given the consent.
- Right to get the details about the results of diagnostic tests and the diagnosis
- Right to get personal dignity and privacy during the examination, procedure and treatment
- Right to accept or reject any treatment lines offered
- Right to get appropriate pain relief and pain management
- Right to obtain any information or document s such as medical reports, etc as documented in the medical record
- Right to participate or refuse in any research activities or any clinical trial
- Right to get information on expected cost of the treatment
- Right to get discharged against medical advice
- Right to get information regarding the bill
- Right to get education about the diet and nutrition
- Right to get educated about immunization
- Right to keep patient information confidential
- Right to raise complaints and grievance
Patient responsibility
- Responsibility to abide by all the hospital rules
- To provide accurate and complete information regarding the present complaints, past illness and hospitalization and other matters relating to patient illness
- Provide complete and accurate information including full name, address and other information
- Responsibility to provide all information about the third party payment
- To follow the prescribed treatment plan and carefully comply with the instructions given
- To follow the doctor order
- responsibility to respect the hospital staff, patients’ and visitors
- responsibility to understand the charter of patients’ rights and seek clarification if any
- responsibility to participate in care plan development and discharge process
- responsibility to inform the changes in the patient condition
- To be on time in case of appointment. To cancel or reschedule inform as far in advance as possible
- responsibility to take care of hospital property (equipment)
- To pay for service billed for in a timely manner as per the hospital policies
- To respect that same other patients medical condition may be more urgent than yours and accept that your doctor may need to attend them first
- Not to give medication prescribed for him / her to others
- To accept the measures taken by the hospital to ensure personal privacy and confidentiality of medical records
- To attend the follow-up appointment as requested
- Responsibility to inform the theft
- Not to take any medication without the knowledge of doctor and healthcare professionals
- To provide correct and truthful history
- Responsibility to raise grievances in case right are not honoured
