Patient & Family Rights

  1. The right to considerate the care with full respect of patient’s dignity regardless of the nationality, colour, age, sex, religion and disability.
  2. Right to select / change the doctor.
  3. Right to know them by name the physicians, nurses and staff members involved in the treatment.
  4. Right to request for a change or transfer to other hospital as per hospital policy.
  5. Right to avail treatment.
  6. Right to get safe environment during treatment.
  7. Right to be free from any form of physical, verbal abuse and harassment.
  8. Right to know the nature and inherent risk of any procedure to which the patient has given the consent.
  9. Right to get the details about the results of diagnostic tests and the diagnosis
  10. Right to get personal dignity and privacy during the examination, procedure and treatment
  11. Right to accept or reject any treatment lines offered
  12. Right to get appropriate pain relief and pain management
  13. Right to obtain any information or document s such as medical reports, etc as documented in the medical record
  14. Right to participate or refuse in any research activities or any clinical trial
  15. Right to get information on expected cost of the treatment
  16. Right to get discharged against medical advice
  17. Right to get information regarding the bill
  18. Right to get education about the diet and nutrition
  19. Right to get educated about immunization
  20. Right to keep patient information confidential
  21. Right to raise complaints and grievance

 

Patient responsibility

  1. Responsibility to abide by all the hospital rules
  2. To provide accurate and complete information regarding the present complaints, past illness and hospitalization and other matters relating to patient illness
  3. Provide complete and accurate information including full name, address and other information
  4. Responsibility to provide all information about the third party payment
  5. To follow the prescribed treatment plan and carefully comply with the instructions given
  6. To follow the doctor order
  7. responsibility to respect the hospital staff, patients’ and visitors
  8. responsibility to understand the charter of patients’ rights and seek clarification if any
  9. responsibility to participate in care plan development and discharge process
  10. responsibility to inform the changes in the patient condition
  11. To be on time in case of appointment. To cancel or reschedule inform as far in advance as possible
  12. responsibility to take care of hospital property (equipment)
  13. To pay for service billed for in a timely manner as per the hospital policies
  14. To respect that same other patients medical condition may be more urgent than yours and accept that your doctor may need to attend them first
  15. Not to give medication prescribed for him / her to others
  16. To accept the measures taken by the hospital to ensure personal privacy and confidentiality of medical records
  17. To attend the follow-up appointment as requested
  18. Responsibility to inform the theft
  19. Not to take any medication without the knowledge of doctor and healthcare professionals
  20. To provide correct and truthful history
  21. Responsibility to raise grievances in case right are not honoured
